Hi! I just got my fiancee visa but i had problems with my name. My birth certificate is using Ma. Fatima instead of Maria Fatima. The name Maria was used in all of the passports and US visas i had through the years. Thanks to that dumb travel agency who put that name Maria Fatima in my passport! My only question is, why is it until now we get the old entries in my grand daughters birth certificates wherein the decisions and certificates of finality from the court had been received by your office. The decisions were annotated in the old birth certificate since 2001. When we got copies of the birth certificates just last Oct. 15, 2010 they have not change the entries in the birth certificates yet. Here is the Registry Nos.

Magandang araw po! This is all about sa epiyyedo ko nung dalaga pa ako(maiden name). ORLANES po kasi ang ginagamit ko since primary up to college not until nang kumuha ako ng NSO birth certificate na instead na Orlanes yung nakalagay dun, Orlanez po yung naka-record. And aside from that, Yung birthday na ginagamit ko is March 23, 1976 then yung nasa NSO ko is March 20, 1976. So ngayon po, ang mga documents ko halos lahat ay HINDI nakaayon sa name and birthdaate ko sa NSO. Ano po ba ang pwede kong gawin to correct this?.. For now po, and ginatamit ko na eh ‘yung NSO base ko.

Hi, year 2006 nagfile ako ng petition for correction for my first name from “Joesiline to “Jocelyn” and it was approved kaya po nakakuha ako ng birth certificate with corrected name Jocelyn i use it for passport application that was year 2007. so i got the passport. Good Morning po. Gusto ko pong malaman kung posible po ba na walang middle name ang isang citizen at anu-ano ang maaaring consequences naman nito kung sakali?
Wala po nakaindicate na Middle Name sa Birth Certificate ko. Pero nakagawian ko pong isulat ang middle name ng mother ko mula pa nung bata ako sa mga school papers. Hindi po sila kasal ng father ko at hiwalay na sila. Kasalukuyan po akong kumukuha ng B.S. Pharmacy at ginagamit ko po ang middle name ng mother ko. Mag-aapply po sana ako para sa passport at naging question ang middle name sa mga IDs at nbi clearance ko na wala naman po sa Birth Certificate ko. good morning. last may i forwarded my corrected fullname to the nso main office for registration and authentication infact i already received the return card, however, until at present when i requested for an authenticated copy of my birth certificate here in our place in cagayan de oro city, what they gave me is still the authenticated copy of my birth certificate where the erroneous entry still appears. may question is, gaano ba talaga katagal mag correct ng birth certificate considering that the i submitted my corrected bitrh certificate last may.

I have concern reagrding my birth certificate it is all about the gender : instead of MALE it became FEMALE and My name is J HANNIS thats a way in affairing in my live birth certificate. I was born in the house only. The person that reported my data at Loca Registra Office was dead already. What is the solution that i will work out so that can I apply for a job. I dont have any documents that could support my identity in applying. This is not my fault that my name was wrong in entry.

hi! here’s the situation., the birth certificate of my 2nd daughter was registered with her father’s surname (acknowleged and notarized last jan. 18, 2011). But since we are NOT living-in together and NOT married (and never getting to), i want to change it to my surname so as not to cause any problems in the future (such as insurance, bank accounts etc.) above all, not to cause psychological prob with my first daughter since she bears my surname. I went to the city hall where it was registered before they pass it to NSO but i was told that it’s not possible anymore. is it true? is there any way I can change it? Anyway aside from we’re not married, the father isn’t annuled from his wife neither legally separated. (which i knew only recently). Hello po. malaki ang problem ko sa aking birth certificate nso copy ang gender ko po doon ay male pero sa local registrar copy ay tama naman na female. Lahat po ng tinanong ko eh ang kailangan daw i file ko sa court. napakalaki ng presyong hinihingi sa akin dahil malaki ang bayad sa attorney. How would I go about changing that process. I have been using my dads last name and my moms last name as my middle initial ever since I was a child.
