I have a problem about the right spelling of my name. In my LCR it was spelled as Hazzelle and so with my Transcript of records in college. Furthermore, I found out that it was spelled in my nso birth certificate as Hazielle. Sir/Madam, for your information, I am used to spell my name as Hazelle, and it is the one I used in my voters ID. My birth certificate from NSO has the wrong spelling of my middle name. Instead of Rellon, it has Rillon.
